Help Identifying Orchestral Music Box

Jeff Adkins wants his music box identified, and he gives a short
description of it, saying that he has the tune card, which he hears may
identify the maker.   He is right about that.  I have the complete set
of H.A.V. Bulleid's "Musical Box Tune Sheets."  That booklet and its
three supplements is the chief resource for identifying tune sheets.

If Jeff will send me a good, clear picture of his tune sheet, I can
probably help him, and possibly even approximate its date of manufacture
from dating the tunes on its cylinder.  Anyway, it will be fun.
