Vaclav Slavik Barrel Organ

I recently restored a reed playing monkey- or hand-organ with
a medallion on the front that reads:

 VACLAV SLAVIK
 HONOVICICH
 HOTOVITEL HARMONIK

It has a pinned barrel and plays 28 notes.  It appears to be from the
turn of the century, or earlier.  You can see pictures and specs at

It came with its original shipping crate which has a paper shipping
label with the following info:

 69  Von Horovic nach Zbirow (Haltestelle) der Boehmichen Westbahn

Is anyone familiar with this organ or the firm that made it?  I am
unable to find anything about it in any of the references that I have.
