Crediting Composer & Lyricist

This isn't exactly a mechanical music question but it relates to music.
When crediting the composer and lyricist for a song, what is the proper
wording, grammar and capitalization?

  Music by J. Smith and lyrics by J. Jones.
or
  Music by J. Smith.  Lyrics by J. Jones.

If the composer and lyricist are the same, should it be

  Music and lyrics by J. Smith
or
  Music and Lyrics by J. Smith

I know this is a nitpicky question, but since I sell both piano rolls
and sheet music, I would like to know the proper wording to credit the
composers and lyricists.
