Seek "Ned Wayburn's Original Charleston" Chart

I am hoping someone could send me a scan, digital photo or Xerox of
the "chart" which accompanied Adam Carroll's Ampico roll 207331-F,
"Ned Wayburn's Original Charleston."  In lieu of lyrics the roll
contains two color (red _and_ green) counts (one-and-two-and etc) as
well as indications of dance motions.  The movements correlate to an
instruction sheet which the roll suggests the listener to reference.

I recently acquired an original roll (which I brought back from the
dead).  The box was intact but the instruction sheet was missing.
I'd appreciate anyone letting me know if they'd be willing to make
me a copy.  I'd happily pay the postage.
