We moved a whole collection of pianos from UK to Canada a year ago.
You do need CITES forms, and you can include several instruments on one
form.  It was left to us to decide which pianos had ivory key coverings
(Steinway does, some cheaper makes have Ivorine, which is essentially
plastic and therefore is irrelevent to CITES).

You also have to specify the species of the ivory.  They were happy
with Loxodonta Africana (African elephant) and there is only one small
fee to pay however many pianos are shipped.  (They also want to know
the address at which you are going to keep the elephant!)

If you are shipping more than one species of ivory, an additional fee
is charged for each species.  It was all perfectly straightforward for
us, our shipping firm locating the forms.  Good luck with the move!
