Converting Pianomation CD to MIDI

I have a 1997 PianoDisc 128+ with floppy drive.  Over the years I have
used Giebler software to convert Disklavier and PianoDisc floppy disk
data files to SMF [Standard MIDI File] format and store them on my PC
hard disk drive.  QRS floppies were easy to save because they were
already in SMF format.

Now I would like to buy some QRS CDs, which I am told store the piano
data in analog audio format.  How can I extract and convert that data
to SMF format?  I'm sure that question is easier to ask than it is to
answer.
