Converting MP3 or WAV Audio to MIDI

Greetings and Happy New Year! Did someone say they could make a MIDI
file from an MP3 or WAV audio file? How accurate is the result?
I guess if there's no reverb or echo it might be more accurate.

Also, does anyone scan sheet music and convert it to MIDI? I have the
Tchaikovsky "Francesca da Rimini" for piano duo which I would eventually
like to have on a piano roll. Right now QRS is charging $40 to perforate
a single roll. The Francesca piece would take two rolls.

Which music scanning program is accurate and easy to use? I might do it
myself.
