John, that looks like an early-style Holcomb and Hoke Electramuse
wallbox, for a non-selective rare jukebox ca 1927-1928.  For
comparison, I've included a photo of a similar Electramuse at
the Orchestria Palm Court Restaurant in San Jose, California.
