Responding to the questions about the Regina a couple days ago:

Any musical box that doesn't immediately start probably has a governor problem. This can usually be remedied with a good cleaning and lube. However, the problem could also stem from bent or missing teeth, a bent gear stem, etc. Considering the value of the instrument, it would be prudent to have a competent restorer look it over.

As to the on/off lever needing to be held on for a second to get it started. The reason for this is because the Regina has a detent on the spring barrel which needs to be cleared at the start. This detent shuts it off at the end of the tune and therefore needs to be cleared at the beginning. If the lever is kept in the on position the disc will continue to play until the spring is wound down.
