I was in my attic and located a Holtzer-Cabot electric motor.
110 VAC, 3.2A, 60 Hz.  1150 rpm, Frame OS 16, Motor # 555791.
Mounting hole centerlines: left-to-right 4-1/2",  front-to-rear 4-3/4".

Looks like it came from a player air pump.  Can't say for sure.
Condition: excellent visual, excellent running.  Has small double pulley
on shaft.  No noticeable wear.
