MIDI Files Created from Piano Rolls

Robbie,  Can you direct me to a book, article, etc., which
addresses the creation of MIDI files from piano rolls?

I know a lot about player pianos but would like some kind of
description of how MIDI files are created!  Starting with:
What is a MIDI file?  E.g., what do the initials stand for?

 [ specification encompasses both the wireline data transmission
 [ definition and the Standard MIDI File (SMF) data storage format.

 [ or a real piano equipped with a recording system.
