Formatting Floppy Diskettes for PianoDisc
By Gary Davis

If you are having problems formatting low density, older format floppy
disks with a modern version of Windows then reboot your PC with a DOS
6.2.2 floppy boot disk. (You may have to change the boot order in your
BIOS setting to look at the floppy drive first.)

If you can't find any old single-density disks you can fool the disk
drive into accepting a high density disk as a low density disk by
putting a bit of tape over the extra square hole on the edge of the
floppy (not the write-protect hole, the one on the left side as you
look at the front of the disk.)

If you don't have a DOS boot floppy there are disk images are available
on the Internet with a Google search, (probably a technical copyright
violation, but I won't tell Bill Gates!). Hope this helps...
