Chart of MIDI Note Numbers

Thanks to all who offered their assistance in finding a chart of
note names, MIDI numbers and frequencies.  I located the chart via
Google, courtesy of The University of New South Wales, Sydney,
Australia.  For anyone else who might have the need, here is
