Loud Pedal MIDI Encoding

Yamaha Disklaviers and QRS players use controllers 64 and 67 for
sustain and una corda, like most synthesizers.  Pianos that allow
MIDI control of the sostenuto pedal use controller 66.  These
controllers are written into the MIDI specification.
