This is the page with the Chickering Decal.  I think the less
ornate decal would be the one you wanted.

Usually there is a fairly small decal saying "The Ampico" just
above the hinge of the fallboard on the right.  The one you want
is http://www.decalsunlimited.com/collections/fallboard-1?page=3 
number 2010 [3/4" x 2-1/4"] unless there is a smaller one.
