For those of you who have shipped large crated items to Europe
(e.g., Vorsetzers, cabinet coin pianos, floor model music boxes,
etc.), what are the best options for cost effective shipping?

I'm considering a piece to go from Chicago to Switzerland, as well as
a piece to go from Chicago to Norway. Do you crate and pack the items
yourself or is there a service that does that?

If I were to crate myself, and deliver to the freight (airport) 
terminal,
do the major airlines offer freight service on their passenger jets?
