I am in the UK and had no problem getting an English copy of this book
direct from Huismuziek.  Email them at info@huismuziek.nl.geentroep
[delete .geentroep].

The book was 18 Euro plus postage.  The only problem you may have is
getting the money to them.  I ended up buying Euros and sending cash.
