John Philips wrote about a grub screw failing to hold a lever onto a
shaft.  It might be worth trying a small piece of copper in the hole
before inserting the grub screw.  Problems can be caused by a hardened
shaft; the copper will distort under pressure of the screw and will
grip the shaft better than a screw.
